A laboratory clerical check of all documentation relating to the pre-transfusion sample is performed. Pre and post- transfusion venipuncture samples are visually inspected and compared for the presence of hemolysis. Preliminary tests may include the determination of patient's ABO group, Direct Antiglobulin test (DAT) and a screen for the detection of atypical antibodies. Additional testing is performed based on the patient’s clinical symptoms and/or laboratory findings.
Minimum of two (2) –6-7 ml EDTA (lavender) tubes, unless pediatric patient, mixed thoroughly by gentle agitation.
Label specimen with the required minimum information: patient’s last name, first name, PHN or unique identifier and date of collection.
Complete Requisition (must include):
Patient’s last name, first name, date of birth and PHN or unique identifier
Facility name, complete address, phone and fax number
Phlebotomist ID information
Date of collection
Transfusion Reaction Details
Reaction date, clinical signs and symptoms, pre & post vitals, measures taken
Blood component blood group, component type, donation number, volume transfused, transfusion start and end dates and times
