Correct Canadian Blood Services Stem Cell Registry search identification numbers (ID#) and International Registry ID#'s must be indicated for both patient and donor/cord(s) where applicable.
Please note for cord procurement requests, the Request for Shipment of Cord Blood Unit form must be completed. Canadian Blood Services Stem Cell Registry will accept donor work-request forms from International Registries, as long as the following information is provided:
- Prescription for Marrow, PBSC or Leukocytes
- Patient Status Report
- Verification of Patient and Donor Typing
