What you need to know before completing this questionnaire

Before proceeding to the questionnaire, please make sure that these statements are true:

  • I have not been to the dentist for a cleaning or filling today. I have not been to the dentist for an extraction, surgery or root canal in the last three days. (This does not cause a deferral if you are donating plasma.)
  • I have not had a tattoo or piercing in the last three months.

If ANY of these statements are false, you will NOT be eligible to donate. Please rebook your appointment.

Will this questionnaire let me know if I’m eligible to donate today?

This questionnaire will not let you know whether or not you are eligible to donate today.

If you answer ‘yes’ to any of the questions*, you need to clarify your eligibility to donate before coming in. Consult our ABCs of Eligibility or contact us at 1 888 2 DONATE (1-888-236-6283) or webchat. Final eligibility will still be determined by staff at the donor centre.

In addition to completing the questionnaire, please review the current COVID-19 wellness check questions. If you answer ‘yes’ to any of these questions, you’ll need to reschedule.

*Note that you do not need to contact us if you answer ‘yes’ to the question ‘Are you feeling well today?’. By completing this questionnaire, you are agreeing that you are the person who will be donating.

What happens after I complete the questionnaire?

Once you have completed the questionnaire, a QR code will be generated for your donation appointment. If you are completing the questionnaire on your phone, you will be able to access your Q-pass QR code on the GiveBlood app. You can also print a physical copy of your Q-pass QR code to bring with you.
